Treating Global Health At Your Doorstep Starts with a Good Patient History: The most potent, cost-effective, and accurate diagnostic tool that we have even in our advanced age of technology remains an accurate and comprehensive linguistically and culturally appropriate patient history. It has been critical in the advancement of medicine; however, in some cases there is an overdependence by clinicians on diagnostic technology which may impede a fuller understanding of the circumstances of patients in the exam room. The most potent, cost-effective, and accurate diagnostic tool that we have even in our advanced age of technology remains an accurate and comprehensive linguistically and culturally appropriate patient history. In this webinar, Dr. Ed Zuroweste and Dr. Laszlo Madaras, who collectively have over 50 years of practice in primary care, ask the question, “Has it become so normal to ask for tests for the most basic assessments that part of the art of medicine is being lost?” The presenters will explore the value of taking a culturally and linguistically appropriate history from the patient together with a thorough – and focused – physical exam. Spending a few minutes with the patient asking open-ended questions may save time and money by: getting the correct diagnosis and treatment plan; preventing expensive, unnecessary, and potentially harmful tests; reducing the number of specialists who may not need to see the patient; and even possibly improving patient satisfaction. Turnover Calculator Tool (2020). Resource Type: Toolkit. Description: Health centers can access this tool on a rolling basis, completing their turnover information from the year prior. Once adequate information has been collected, ACU will be able to aggregate the data on a state, regional and national basis, which can be shared to assist health centers in benchmarking their own data. Description: As health centers are held accountable for cost containment and high quality of care under value-based pay arrangements, it is increasingly important for providers to document the social determinants of health (SDoH) barriers of their patients and what services they are providing (both clinical and non-clinical) in order to properly care for those complex patients. Two-Tiered Screening for Patient Social Needs in a Safety-Net Setting: Using Two Pre-Screening Questions to assess patients’ social needs at a Chicago-area FQHC (2022). Resource Type: Template. Description: Erie Family Health Centers, a Chicago-area Federally Qualified Health Center, initiated a two-part social needs screening process with patients in May 2020. A medical assistant asked a single prescreening question about interest in being connected to resources and interested patients received a follow-up call from staff to screen for needs and connect with resources. A second prescreening question was added asking patients their preferred method for receiving resources.
